Palatal expansion was always a challenge especially in adults.The mature bone makes it more difficult for the orthodontist to bring about the correction in an adult.However SARPE [Surgically assisted palatal expansion]is one possible option which has been in practice sinse its introduction in 1938 .Where the maxilla is split in the middle by surgery 1st and then an expansion appliance is used to bring about correction.Case report-22 yr / Male .chief complaint of decreased airway.Treated with DR.Keshav ,for SARPE.in 2010.
